Community connection a path to wellbeing

A male Aboriginal Elder with multiple chronic health conditions was reluctant to engage with healthcare providers or leave his house.

His Integrated Team Care coordinator encouraged him to attend a men’s group to share like experiences with other men who were actively managing their health needs. The client was extremely happy to reconnect with old friends, which gave him a sense of community connection and increased his well-being and self-esteem.

When the client heard a diabetes educator speak to the group, he was surprised at how much he learned and decided to actively take part in his care.

The Integrated Team Care team quickly re-engaged with his GP to have his GP Management Plan updated and obtain current referrals, allowing him to access supplementary services.
